Explanation of services:

Home health care (Medicaid PA) – Licensed RN to provide hands on care and oversight; licensed HHA to provide care and assistance with bathing, dressing, mobility, medication reminders, meal planning and preparation, and maintaining safety.

Personal care (Medicaid waiver) – Home care assistant to provide hands on care with meal preparation, light housekeeping, community transportation to grocery store, pharmacy, doctor’s appointments, and social activities.

Integrated health care coordination (Medicaid waiver) – RN case manager works directly with Medicaid Aged and Disabled Waiver case manager to provide you integrative health care coordination.

Injury & surgery restorative care (Medicare/Workman’s comp) – Therapy and nursing staff work with you and your doctor to develop a restorative, short-term plan of care to return to your prior level of function.
